FS#39 - <FIELD> tag suggestion

I think that the <FIELD> tag should include a reference to the "Fn" element of the <REC> tag. For example:

<FIELD NAME="id" REC="F1" TYPE="6" LENGTH="4" DEC="0"/>

Where the REC element explictly references the data element in the <REC> tag, rather than trusting the order. While we trust EpiData to always be consistent in creating the database, this provides better internal documentation.
